NOTICE OF COVID-19 DIAGNOSIS

As per the Amended Order of County Judge Clay Jenkins Regarding Long-Term Care Communities, dated April 1, 2020, this notice is intended to communicate that Heritage Gardens Rehabilitation and Healthcare learned on June 20, 2022, we had 1 residents and 0 employees test positive for the COVID-19 virus.
