NOTICE OF COVID-19 DIAGNOSIS

As per the Amended Order of County Judge Clay Jenkins Regarding Long-Term Care Communities, dated May 20, 2020, this Notice is intended to communicate that Heritage Gardens Rehabilitation and Healthcare learned July 3, 2020, that one of its residents has tested positive for the COVID-19 virus. The resident is no longer at Heritage Gardens, and was last in the facility on July 1, 2020.
